Confidentiality
Confidentiality is seen by many as the most important issue when considering a referral for medical services. Students are assured of confidentiality when they request services through the Student Health Center. Confidentiality is maintained and services are provided in accordance with state and federal law. All information obtained in consultation is maintained in a secure file and no information is designated for the students permanent educational record.
The Student Health Center does not routinely notify parents, faculty, or administration or others regarding your care. We are unable to discuss anything relating to student's health care matters unless the student has given written consent.
Our Pledge Regarding Medical Information
The Student Health Center understands that information about you and your health is personal. We are committed to protecting health information about you. This notice applies to all of the records of your care generated by the Student Health Center, whether made by Student Health Center personnel, your personal doctor, or mental health professional.
We are required by law to:
- Make sure that health information is kept private;
- Give you this notice of our legal duties and privacy practices with respect to health information about you; and
- Follow the terms of the notice that is currently in effect.
The detailed Notice of Privacy Practices includes how the Student Health Center will:
- Obtain your personal health information
- Use or disclose your personal health information
- Disclose your personal health information to business associates
- Inform you of your health information rights, including:
- Inspect and copy your health information
- Request to amend your health information
