Medical Records
Your medical record is confidential. If you need to have your records released, the Student Health Center, as required by law, must obtain a written release. This release allows us to transfer your records. A separate release is required for each transfer. Requests should be submitted within a reasonable time to allow for duplicating and mailing.
To have information from your medical record released:
-Complete and sign the Student Health Center Release Form
-Specify information to be release
-Return form and appropriate fee to:
